  • Together with the renewal, the addition of Chris Carmack (Atticus Lincoln), Greg Germann (Tom Koracick), and Jake Borelli (Levi Schmitt) to the main cast was announced.
  • This is the first season since season 10 for which ABC released promotional photos for every member of the cast.
  • On March 12, 2020, it was announced that due to concerns about the coronavirus pandemic, production had been shut down. This hiatus in filming came after the completion of episode 16.21 and was expected to last at least two weeks.[source?]
    • On March 27, 2020, ABC announced that it would not resume production on season 16 and the finished 21st episode would act as the season's final episode.[source?]
    • This decision makes season sixteen the fourth shortest season of the series, only behind season one with 9 episodes, season four with 17 episodes and season seventeen with 17 episodes.

* There was no table read for this episode to ensure that details of Alex's exit would not leak. The few main actors who appear in the episode just showed up to set to film their scenes and were only told then what happened to Alex and what exactly they would film that day. Filming took place on separate days for each actor whose character received a letter. As such, Ellen Pompeo only knew what Meredith's letter said.

  • All voiceovers for the season are narrated by Meredith Grey with three exceptions. Fight the Power and Sign O' the Times were narrated by Miranda Bailey and Richard Webber, respectively. As such, all three remaining originals have narrated a voiceover this season. Additionally, Look Up Child was narrated by Jackson Avery, marking the first time since (Don't Fear) the Reaper that Ellen Pompeo did not appear in an episode in any capacity.
  • The season sees the departure of three main actors: Giacomo Gianniotti, Jesse Williams, and Greg Germann.

* This is the first episode of the series to feature Richard Flood and Anthony Hill as part of the main cast. Flood recurred as Dr. Cormac Hayes throughout the sixteenth season while Hill made one guest appearance as Dr. Winston Ndugu in the episode Love of My Life. It can be presumed that Winston was set to appear in more episodes which ended up being cut from the season due to the coronavirus pandemic.

He made his last appearance in the season eleven finale You're My Home. Dempsey is credited as a guest star for this episode, marking the first time in the show that he is not credited as main cast.

* The beach scenes with George were filmed in mid-October 2020. Krista Vernoff wrote most of the beach scenes herself since the majority of the writers did not know about the special guest stars appearing on the beach. The final scene where Richard and Bailey appear on the beach with Meredith and George was pitched by Meg Marinis and Andy Reaser, who read Krista's beach scenes and saw an opportunity to create an iconic moment by reuniting the four characters.

    • Ellen Pompeo signed a new deal for the season with another salary increase, also upgrading from co-executive producer to executive producer. The contract renewal talks went over smoothly compared to the renewal talks for season 18, which lasted until May. It was later revealed that the deal included a reduced involvement in the series with her only appearing in eight episodes for the season yet she will continue to narrate the rest of the episodes.
    • Chandra Wilson and James Pickens, Jr. had previously signed a multi-year deal ahead of season 18.
    • Kevin McKidd, Kim Raver, and Camilla Luddington all had signed three-year contracts ahead of season 17, meaning their contracts are up at the end of this season.
    • Kelly McCreary's contract was up at the end of season 18 but it was renewed for an unspecified length of time.
