Derek helps a rape victim while Meredith carries around the rapist's penis. Alex Karev becomes one of Bailey's interns and shadows Cristina for the day. * This is the first episode to use a title sequence. The theme song is "Cosy in the Rocket" by Psapp

A bike race injures several people, one of whom is left brain dead. Cristina and Izzie want to use his organs for organ donation, but they find the organ harvesting process an emotional one. Meanwhile, George deals with an over-flirtatious patient, and Meredith has to deal with Izzie and George moving in, Alex showing off, and keeping Derek at arm's length. * This episode is the first one to explicitly feature an LGBT character.
* When harvesting Kevin's organs, the last organ to be removed is the heart. Typically thoracic organs, i.e. heart and lungs, are removed first, the heart in particular as its viability is shorter than other organs and it must be transported to a recipient quickly.
*Despite this episode taking place in summer, there are several coats hanging in the interns' locker room.

An old part of Izzie's life will be revealed which makes her the subject of hospital gossip and Derek, Meredith, George and Alex have a patient with nails in his head. * This episode marks the first time Richard Webber is seen wearing scrubs.

Meredith realizes she could be sued for making a mistake on a surgery. Her life gets more hectic hiding her mother's secret. George finds a towel in a patient, Izzie throws a party and Bailey catches Meredith having sex with Derek. * Meredith is hoarse throughout the episode.
* Richard mentions that Mrs. Drake underwent this surgery before in '99. But when Cristina is looking at the paperwork from the original surgery, it says the surgery date was 10-18-2000.
6 6 "If Tomorrow Never Comes" - All of the interns are in awe when a woman comes in with a record breaking tumor, but it is so far advanced that the woman's prognosis is not good. Now that Bailey knows about Meredith and Derek's affair, their jobs are in jeopardy, and she strongly advises Meredith to end it. Meanwhile, Cristina and Burke's secret relationship gets more complicated, and when no one else can be reached, Izzie has to perform a major procedure on her own. * Richard Webber does not appear in this episode.
* When Alex walks into the gallery, he talks to another intern, who mentions that George and Cristina have been retracting the tumor for eight hours. This-- and the next shot, where they are shown in the rigid retraction positions-- indicates that they haven't moved freely in hours. However, while Alex is talking to the other intern, George can be seen moving around clearly in the OR below.
7 7 "The Self-Destruct Button" - Cristina discovers she is pregnant. George finds out the anesthesiologist has been drinking on the job, but when he tells Shepherd he is thrown out of surgery. George and Izzie catch Derek leaving the house in the morning after he sleeps with Meredith, and when asked who it was, Meredith says it's no one they know. Izzie and George think Meredith is trying to get Derek to favor her, and try to ice her out. * Richard Webber does not appear in this episode.
* In this episode, Izzie is overwhelmed when she finds out about Meredith sleeping with Derek; nonetheless, in the pilot episode, when Cristina is mad at Meredith she yells at her how she slept with her boss, and Izzie is present in the conversation, so, she would already have to know about Meredith's fling.
8 8 "Save Me" - Meredith and Derek grow closer, Alex and Burke argue and perform a risky procedure, Izzie treats a psychic while Cristina treats a pregnant patient. * Richard Webber does not appear in this episode.
* This was the episode submitted for Sandra Oh for the 57th Primetime Emmy Awards under 'Outstanding Supporting Actress In A Drama Series'.
9 9 "Who's Zoomin' Who?" * Kate Walsh as Addison Montgomery Burke operates on his friend. George gets syphilis from Olivia, Izzie and Cristina become body snatchers, Meredith finds it harder to look after her mother and Derek operates on Richard's brain while someone from his past crops up. * Actress Sandra Oh (Cristina Yang) pronounces the patient's disease as "hematomachrosis" while breaking the news to the family, as opposed to the actual name of the disease, hemochromatosis.

Meanwhile, Addison is still roaming the halls of Seattle Grace, prompting Derek to bring in Richard's wife. * Izzie states in the episode that, "The liver is the only organ that regenerates itself," which is false. The skin is the biggest organ of our body and it too regenerates itself. Of course it's possible she was speaking in terms of the connotation of organs inside of our bodies.
12 3 "Make Me Lose Control" * Kate Burton as Ellis Grey (1/6) Secrets are let loose as Meredith's mother is admitted to Seattle Grace and Cristina's pregnancy becomes public when she collapses in the OR. * When Addison and Derek are in the room with the premature baby addicted to narcotics, she is listing options for her and Derek's relationship. The chair that Derek is sitting on has a blanket over it and it changes position throughout the scene many times.
13 4 "Deny, Deny, Deny" * Steven W. Bailey as Joe (2/11)

Cristina struggles to stay in bed when she could be performing surgeries. Alex discovers that he needs to re-take a test to stay at SGH, Derek must choose between Addison or Meredith and Meredith's mother continues to bother the staff of Seattle Grace. * There is a continuity error when Bailey mentioned that Jeremiah Tate was her patient was at least five years. This is impossible as Bailey was a 4th year surgical resident.

Cristina tells a pornographic story. Burke asks Cristina if they're in a relationship. Meredith decides she is out of Derek's life. Derek and Meredith search for a shaman. When Alex chokes, George performs open heart surgery in an elevator, and Izzie has a patient who has had a heart attack on the same day for the past seven years. * Cristina tells a patient a sexual story to replace his porn when the power went out. The story resembled George's fantasy in It's the End of the World.
* This episode is the first time the phrase, "The carousel never stops turning." was used. It would later becoming a recurring element in the show.
* When George and Alex were receiving Dr. Burke's instruments, when the camera focuses on Alex, he does not have a surgical gown, but when the angle of camera changes, he has it back on.
15 6 "Into You Like a Train" * Steven W. Bailey as Joe (4/11) A train crash brings in many patients to Seattle Grace, including Meredith, who is too drunk to work; while the rest of the interns face their own professional crashes as Cristina searches the entire hospital in search of a foot for the Chief; Izzie is unsure how to react around Addison; and Meredith, George, Bailey, Derek, and Burke treat two strangers impaled on a pole. All the while, Meredith awaits Derek's decision. * This episode marks the introduction of Paramedic Ray Sutera.
* George's line, "Am I invisible?" was ad-libbed by T.R. Knight.
* Alex refers to Addison as Satan's whore. The original script said "bitch", but Justin Chambers felt like that was too generic for Alex, so Krista Vernoff had her assistants make up a list of non-generic alternatives. They then chose Satan's whore.
* After Bonnie's pulled off the pole and laid on a table, there was a huge pool of blood under the table to show that Bonnie was actually bleeding out, but ABC wouldn't allow the crew to show it in the episode.
* At the 58th Primetime Emmy Awards, Krista Vernoff received a writing nomination for her work on the episode's script.

16 7 "Something to Talk About" * Steven W. Bailey as Joe (5/11) The hospital staff invade the privacy of a man with male hysterical pregnancy and Meredith is the only person who sympathizes with him, while Alex reveals his own privacy to a patient with a sheltered life. * Kate Walsh joins the main cast as of this episode.
17 8 "Let It Be" - Derek and Addison's personal and professional lives collide as friends visit from Manhattan for more than just a visit as they want radical pre-emptive surgery to prevent cancer, while Cristina and Burke attempt a "normal" date, with surgical consequences and George is left puzzled about a man who fell five stories. * When Meredith sits in her car the Hello Kitty on the bandage is upside down. When she is in the elevator, it is the right way again.
* According to the Official Grey's Anatomy Podcast, the events of Callie's flashback, including her fixing legs warped by polio, take place during this episode.
* Strangely, Bailey is offered fellowships in this episode. This is contrary to the later episodes in which Meredith and her fellow residents choose their specialties. First of all, general surgeons don't have to complete a fellowship, as having completed the surgical residency qualifies a physician as an attending general surgeon. Additionally, Bailey is only in her fourth year of residency. It's later proven in season eight that residents who do want to specialize choose their fellowships in the fifth year of residency.
18 9 "Thanks for the Memories" * Steven W. Bailey as Joe (6/11) None of the doctors can resist cutting as Izzie tries embrace the season by making a perfect thanksgiving dinner but doesn't know how, however Burke saves the day and helps her, while Cristina, Meredith, and Alex hide at work instead of helping Izzie; and George has to embrace his own O'Malley tradition by shooting a turkey. * The dialog between Burke and Izzie when they are cooking dinner is almost exactly the same as George's appendectomy in the pilot. It even had the same background music.
* This is the first Grey's episode to not open with the theme song.
19 10 "Much too Much" - It's a high profile day for Seattle Grace, high profile names become staff and high profile cases arrive, specifically the arrival of Dr. Addison Forbes Montgomery-Shepherd and the mother of quintuplets, in which three of the five are surgical cases. Meanwhile, the rest of the interns face their own low profile dilemmas, as Meredith's one night stand shows up at the hospital more than pleased to see her, Cristina gets keyed and Alex makes a major mistake, both professionally and romantically. * The close up shot of the document she signs reveals Addison's full name, Addison Forbes Montgomery, for the first time. We later learn in PP that Forbes is just one of her two last names and her actual full name is Addison Adrienne Forbes Montgomery.
20 11 "Owner of a Lonely Heart" * Rosanna Arquette as Constance Ferguson A prisoner swallows razor blades to get out of solitary confinement, annoying Cristina in the process. While watching over the quintuplets, Addison teaches a lesson to Izzie using an old technique that Richard taught her long ago. -
21 12 "Grandma Got Run Over By a Reindeer" * Steven W. Bailey as Joe (7/11) Tis the season to be jolly, as Burke and Cristina's religious differences are highlighted when they treat a boy who is refusing a heart transplant, while Derek has little religious problems with his patients of mixed Jewish and Christian family beginning for Chrismukkah while he feels less than festive, while Alex has problems with his own faith in himself as the rest of the interns try to help him study for his exam. * At the end when Cristina returns to Burke's apartment and sees the Christmas tree and the Menorah, Burke is seen reading "Exploring Judaism" in bed.
* When Derek talks to Addison in the middle of the episode, he puts his headphones in his pockets. When he stands up, they are hanging around his neck again.

The Chief enforces the 80 hour work limit with an iron fist so Meredith has free-time, and so does Cristina, but she chooses to go to Twin Falls, Idaho with Bailey for a heart retrieval while the Chief asks Derek for a favor regarding Ellis Grey. George has a teenager who is actually different while Alex treats a man who has a bezoar and Izzie works on Denny Duquette. * This episode marks the first appearance of Jeffrey Dean Morgan as Denny Duquette, Jr.
23 14 "Tell Me Sweet Little Lies" - George is stuck with an elderly lady who refuses to leave, or even refer to him as Dr. O'Malley. While Alex has to wrestle with a Japanese competitive eater to stay in the hospital, all the while being too nervous to open his board results. Cristina is blunt to a guitarist whose fingers need replantation, while lying to Burke about moving in, as Meredith has a euphoric patient she thinks is lying. Bailey's pregnancy is now far enough to cause more problems than fun for her. -

The nurses's strike causes chaos in the hospital, as Meredith accidentally intubated a DNR patient, and then must let her die; Izzie treats a young pregnant patient with much more in common than the patient realizes. George joins the picket line, while the rest of the interns meet their new resident, the anti-Nazi, which Cristina immediately clashes with over her choice of medical care. * Miranda Bailey does not appear in this episode.
* The nurses' strike was resolved as the money earmarked for a multi-million dollar surgery robot went to pay for the additional 40 nurses' salaries. Webber mentions a two-year waiting list for the robot, which would fall near the end of Bailey's residency, and he would use that robot in attempt to lure her back into general surgery.

Away from the bombs, the stress gets to Richard, causing him to have a suspected heart attack. * When Meredith was thrown back from the explosion of the bomb, she was played by a stunt woman.

She freaked out when she saw Natalie in the makeup chair as she had no idea Natalie would be on the show.
* In the previous episode, as Denny Duquette was rushed through the ER, the paramedics mentioned him as "a 35 year old Male", but in this episode, Izzie mentioned him as "a 36 year old Male" and he corrected her stating that he will be turning 37 years in 3 weeks.

Surgical fatalities occurs in threes and sevens, at least that's the rumor on the surgical floor. With four fatalities in the morning alone, superstition rules the roost at Seattle Grace, so Addison jujus Meredith for good luck. Burke has George retrieve a scrub cap Cristina is holding hostage, Meredith and Bailey try to convince a friendly stalker to operate and Cristina is annoyed endlessly by her OCD patient while Richard's past arrives in Seattle Grace. * This was Addison's debut of her unique scrub cap; prior to this, Addison had been wearing the standard navy attending scrub cap since she was officially part of the SGH staff.
* The paramedics tell the doctors that Jesse Fannon is 32, but Cristina later tells Derek that he's 34.

Meredith begins knitting with Izzie, while the Chief and Cristina battle it out during a skills class. * Jeff Perry and Laurie Metcalf, who were previously married to each other, both appear in this episode. However, their characters never interact. The couple's daughter Zoe later appears in season nine.

Everyone has a choice, Meredith between two men, Izzie to tell the truth, and Derek and Meredith to choose to put Doc down. * Two of the songs played during this episode ("Grace" and "Chasing Cars") would later be sung by the cast in the musical episode. These songs also later appeared in the episode She's Leaving Home.

In keeping with his spirit of raising the #12 rank, the Chief has Bailey and Hahn perform tumor resection deemed impossible by many other hospitals. Callie tries to find out who she really is.

Mark, Karev and Lexie try to keep a patient alive although her family are fed up of her being "almost dead" every month. Derek and Meredith get a happy ending as Cristina loses her own happy ending. * Sara Ramírez and Jessica Capshaw were listening to "Single Ladies" by Beyoncé when they filmed the dancing scene.

* According to Michael Aussiello, the final scene was originally shot with Denny meeting Izzie in the elevator, however, Shonda Rhimes changed the scene at the last minute. T.R. Knight had shaved his head, assuming that production had finished, and so it was decided to put George in a military uniform.
