(as suggested by Vincent Ramos) Transsexualism is a condition where a person knows and has an identity of the opposite gender to their birth sex. To be accepted in that gender socially, the person will wear clothes to reflect their gender, and may also go through surgical operations (for the genitals, but may also be for cosmetic reasons) to change their sex to be the same as with their gender identity.
The word transsexual should not be confused with words like heterosexual and homosexual (since transsexualism is independant of sexual orientation), but is formed from the two parts trans - to travel between, and sex, for sex.
